Care Operations Manager - Remote

Overview

Pair Team is hiring a full-time Care Operations Manager to lead and support a team of Community Health Workers (CHWs) and Nurses delivering high-touch, relationship-based care to Medicaid members with complex medical, behavioral, and social needs.

In Short

• Lead a team of CHWs and Nurses in delivering care to Medicaid members.
• Ensure quality and consistency across care delivery.
• Drive member engagement and coaching for team success.
• Provide day-to-day guidance and performance support.
• Monitor performance metrics and address gaps.
• Support onboarding and training for new team members.
• Foster a collaborative team culture focused on trust and advocacy.
• Participate in team meetings to improve care delivery.
• Conduct field-based member visits as needed.

Requirements

• Active CHW Certification in Texas (preferred).
• Based in Travis County (Austin, TX) (required).
• Bilingual in English and Spanish (preferred).
• 2+ years of experience managing community-based teams in healthcare.
• 2+ years of experience in care coordination or similar roles.
• Reliable access to a vehicle.
• Strong communication skills and cultural competency.
• Ability to work independently in a remote environment.
• Organized and detail-oriented with digital tools.
• Valid driver’s license and active auto insurance.
