The Bilingual Case Management Specialist at Pair Team plays a critical role in engaging with individuals living with serious mental illness, substance use disorder, and high medical needs, while primarily working remotely with occasional community visits.

In Short

  • Work remotely with 2-3 community visits monthly.
  • Engage with individuals to promote health goals.
  • Utilize evidence-based approaches in case management.
  • Provide social support navigation to members.
  • Conduct telephonic and SMS outreach.
  • Collaborate with a multidisciplinary team.
  • Meet health plan compliance metrics.
  • Assist individuals in accessing community supports.
  • Maintain a caseload of clients.
  • Participate in systematic case reviews.

Requirements

  • 1+ years of general work experience, preferably in case management.
  • Bilingual in English and Spanish.
  • Strong understanding of cultural fluency.
  • High degree of empathy and organizational skills.
  • Ability to work collaboratively in a team.
  • Technical skills with CRM databases and basic software.
  • Valid driver’s license and auto liability insurance.
