Overview

As an essential member of our Special Needs Plan (SNP) care team, the telephonic Care Manager Specialist (CMS) plays a key role in coordinating the care of our members, particularly those with social determinants of health (SDoH) needs and stable health conditions.

In Short

  • Engage telephonically with members to coordinate care.
  • Utilize resources to connect with hard-to-reach members.
  • Develop individualized care plans with members.
  • Maintain documentation in electronic health records.
  • Collaborate with interdisciplinary care teams.
  • Identify and connect members with community resources.
  • Meet regulatory requirements within timelines.
  • Support team objectives and enhance operational efficiency.
  • Conduct oneself with integrity and professionalism.
  • Communicate effectively with members and colleagues.

Requirements

  • 2+ years of experience in a health-related field.
  • 2+ years of customer service experience.
  • Proficient in Microsoft Office Suite.
  • Access to a private workspace for telecommuting.
  • Experience in Medicare and Medicaid care management preferred.
  • Familiarity with community resources and services.
  • Excellent customer service skills.
  • Bilingual skills, especially English-Spanish preferred.
  • Associate’s degree in a health-related field required.
  • Practical Nurse Degree/Certificate or bachelor's degree preferred.
