In Short

  • Integral role in creating a best-in-class patient experience.
  • First point of contact for patients in the Care Coordination team.
  • Manage all inbound communication and assess urgency of needs.
  • Support patients with scheduling visits and improving attendance.
  • Maintain and update patient data and technology troubleshooting.
  • Deliver compassionate, patient-centered communication.
  • Follow defined processes and protocols for consistent care.
  • Work Tuesday-Saturday with specific hourly schedule.
  • Reports to the Patient Engagement Specialist Lead Care Coordinator.
  • Part of a team dedicated to improving patient outcomes.

Requirements
