Case Manager - Remote

Overview

The PRS Case Manager is responsible for conducting in-person home visits, assessing the needs of each assigned child, and making referrals for services for unaccompanied immigrant children released from federal custody.

In Short

• Conduct home visits and phone contacts to assess child’s placement safety.
• Perform ongoing case management and make appropriate referrals.
• Provide crisis intervention services as needed.
• Submit detailed documentation in accordance with agency requirements.
• Utilize social work best practices for assessments.
• Maintain case file documentation and conduct internal chart reviews.
• Participate in team meetings and training.
• Complete timesheets and other administrative records.
• Demonstrate knowledge of the Sanctuary model.
• Travel frequently to visit clients' homes.

Requirements

• Bachelor's Degree in Social Work or equivalent.
• Bi-lingual in Spanish and English.
• 1-year experience in child welfare or social service setting.
• Ability to work shift hours as assigned.
• Valid driver's license and auto insurance.
• Pass a criminal history background check.
• Current CPR and Standard First Aid certification.
• 40 hours of required training per year.
