PRS Case Manager, Home Study & Post Release Services - Remote

Overview

The PRS Case Manager is responsible for conducting in-person home visits, assessing the needs of each assigned child, and making referrals for services for unaccompanied immigrant children released from federal custody.

In Short

• Conduct home visits and phone contacts to assess safety and appropriateness of child’s placement.
• Perform ongoing case management with a focus on screening and making appropriate referrals.
• Provide crisis intervention services directly or connect families to services.
• Submit detailed and thorough documentation in accordance with agency requirements.
• Utilize social work best practices for thorough assessments.
• Maintain case file documentation in accordance with ORR requirements.
• Participate in team meetings and training.
• Carry a caseload varying on program needs.
• Complete timely and accurate timesheets and records.
• Demonstrate a commitment to trauma-informed care.

Requirements

• Bachelor's Degree in Social Work or equivalent.
• Bi-lingual in Spanish and English.
• Experience working with immigrant populations.
• 1-year of experience working with youth in a child welfare or social service setting.
• Must be 21 years or older.
