Overview

The Care Services Manager is responsible for developing professional and community relationships that will enable the Association to provide care and support to those living with ALS.

In Short

  • Manage care services program for individuals with ALS and their families.
  • Conduct specialized care consultations and needs assessments.
  • Actively manage a portfolio of patients ensuring their needs are met.
  • Provide information on services and resources available to patients.
  • Coordinate referrals to ALS Association programs and healthcare providers.
  • Facilitate support group meetings and engage participants.
  • Collaborate with clinic staff to maintain strong communication.
  • Evaluate care services programs for effectiveness.
  • Ensure accurate reporting and adherence to procedures.
  • Assist in establishing community partnerships.

Requirements

  • Bachelor’s degree in social work, health administration, or related area required.
  • 3-5 years of related professional experience preferred.
  • Strong management, problem-solving, and critical thinking skills.
  • Knowledge of ALS and experience with neurodegenerative diseases is a plus.
  • Excellent communication and interpersonal skills.
  • Proficiency in Microsoft Office and experience with Salesforce.
  • Ability to handle confidential information with integrity.
  • Excellent organization skills and attention to detail.
  • Ability to work independently and manage multiple projects.
  • Ability to travel locally and nationally as required.
