A growing national home care organization is seeking an experienced Medicaid Referrals / Outreach Manager to support expansion in Connecticut. This role involves building referral pipelines and managing relationships to drive client growth through Medicaid programs.

• Develop and grow referral relationships tied to Connecticut Medicaid waiver programs

Requirements

• 3+ years of experience with Medicaid waiver programs in Connecticut
• Strong understanding of CT DSS, HCBS workflows, and referral processes
• Proven relationship-building and outreach experience
• Ability to work remotely while managing a local market
• Bachelor’s degree preferred or equivalent experience
