Remote Admissions Coordinator – Long-Term Care & Rehabilitation (RN or LPN)

Overview

A managed care organization serving Kansas Medicaid members is seeking an experienced and compassionate nurse (RN or LPN) to join its Care Management team in a fully remote role.

In Short

• Coordinate and oversee admissions and transitions of care for members entering long-term care.
• Review clinical documentation and assessments for appropriate placement.
• Serve as the primary liaison between health plan and SNF admission teams.
• Manage prior authorization workflows and verify eligibility.
• Collaborate with interdisciplinary teams for safe transitions of care.
• Provide education to members regarding benefits and services.
• Maintain accurate documentation in EMR and care coordination systems.
• Ensure compliance with Kansas regulations and organizational benchmarks.

Requirements

• Active, unrestricted Kansas RN or LPN license.
• 2–3 years of experience in home health or long-term care.
• Knowledge of Kansas Medicaid/KanCare systems.
• Strong communication and documentation skills.
• Proficiency with EMRs and care coordination technology.
• Ability to work independently in a remote environment.
