Overview

A growing national home care organization is seeking an experienced Medicaid Referrals / Outreach Manager to support expansion in the Stamford and surrounding markets.

In Short

• Develop and grow referral relationships tied to Connecticut Medicaid waiver programs.
• Work closely with case management organizations and community partners.
• Educate referral sources, clients, and families on home care services.
• Coordinate with internal teams to ensure smooth start-of-care.
• Track outreach efforts, referral activity, and conversion metrics.
• Attend occasional in-person meetings with referral partners.

Requirements

• 3+ years of experience with Medicaid waiver programs in Connecticut.
• Strong understanding of CT DSS and referral processes.
• Proven relationship-building and outreach experience.
• Ability to work remotely while managing a local market.
• Bachelor’s degree preferred or equivalent experience.
