Care Coordination Assistant (Remote, LPN or MA, Bilingual Spanish Speaking)

This position is responsible for assisting in providing a model of care that ensures the delivery of quality, efficient, and cost-effective healthcare services.

Requirements

• Licensed Practical Nurse (LPN) or Certified Medical Assistant (CMA) required.
• High school diploma required; Bachelor's Degree preferred.
• Five years of experience in a healthcare setting preferred.
• Moderate to high computer literacy required.
• Knowledge of Microsoft Office suite software required.
• Knowledge of performance improvement and documentation requirements desired.
• Experience with motivational coaching techniques highly desired.
• Strong interpersonal skills for effective communication.
• Ability to work autonomously and be accountable for outcomes.
• Must be comfortable in a fast-paced environment.
