Overview

The Bilingual Care Coordinator will support Pomelo Care's clinical practice by managing operational and administrative processes to enhance efficiency in patient care.

In Short

Provide care navigation support for patients.
Input patient data into the EHR.
Monitor and respond to patient inquiries via phone.
Conduct eligibility checks and file claims.
Develop workflows to improve efficiency.

Requirements

Fluent in Spanish and English.
4+ years of healthcare administration experience.
Familiarity with SDOH platforms.
Results-oriented and detail-oriented.
Ability to work cross-functionally with clinicians.
