Overview

The Bilingual Continuous Care Coach is responsible for assessing, coordinating, and managing the care needs of patients with chronic eye diseases, ensuring high-quality support throughout their care journey.

In Short

  • Manage patient caseloads and communicate with patients and their families.
  • Educate patients on self-management and treatment adherence.
  • Assess and address barriers to care using motivational interviewing.
  • Work collaboratively with healthcare providers and community resources.
  • Document health outcomes and maintain care plans.
  • Provide support to up to 15 patients daily.
  • Work within normal business hours across various time zones.
  • Communicate effectively with team members and stakeholders.
  • Ensure a HIPAA-compliant remote workspace.
  • Contribute to the goals of Lumata Health.

Requirements

  • Bilingual in English and Spanish.
  • 2+ years of experience in eye care.
  • Certified Ophthalmic Assistant (COA) or equivalent required.
  • Outstanding written and verbal communication skills.
  • Excellent organizational and time management skills.
  • Ability to work independently and in a team environment.
  • Proficient with Electronic Medical Record (EMR) systems preferred.
  • Strong consultative skills for patient guidance.
  • Reliable internet connection for remote work.
  • Ability to maintain a secure and distraction-free home office.
