Bilingual Care Coordinator - Remote

Overview

Pair Team is hiring a full-time Bilingual Care Coordinator to support our expansion into Chronic Care Management (CCM). This role focuses on providing consistent, relationship-based support for adults managing chronic conditions.

In Short

• Manage a defined caseload of enrolled CCM members
• Conduct proactive outreach to support ongoing engagement and care coordination
• Identify and address social determinants of health
• Coordinate referrals to community-based organizations
• Track referral progress and support members
• Partner closely with RN and NP
• Support appointment scheduling and follow-up coordination
• Maintain accurate documentation of member interactions
• Meet outreach and documentation expectations
• Collaborate closely with the care team

Requirements

• Minimum of 1 year of experience in care coordination
• Bilingual - Spanish and English fluency required
• Experience with Medicare/Medicaid population
• Strong understanding of social determinants of health
• Comfort in structured, metrics-driven programs
• Strong communication skills
• Ability to work independently in a remote environment
• Proficiency with documentation systems
• Reliable internet connection
