Overview

As a Care Coordinator at Devoted Health, you will play a vital role in supporting our care coordination function, advocating for members and ensuring they have the resources necessary for better health outcomes.

• Conduct health risk assessments through calls to members.
• Coordinate with provider offices and home health agencies.

Requirements

• High school diploma or GED equivalent.
• Strong organizational skills and attention to detail.
• Experience in social services is preferred.
• Comfortable working in an interdisciplinary team environment.
