The Community Guide Case Manager plays a crucial role in providing comprehensive care to members by advocating for their needs and connecting them with necessary resources.

  • Serve as an advocate for members and their caregivers.

  • Identify and catalog community resources available to members.
  • Assist members in applying for government benefits and programs.

  • Work collaboratively with internal teams at Devoted.
  • Demonstrate reliability and timeliness in member interactions.
  • Support either Mountain or Pacific time zones.
  • Adapt to a fast-paced, start-up environment.

Requirements

  • Minimum of 2 years in case management or care coordination.
  • Experience in health insurance, preferably Medicare Advantage.
  • Strong verbal and written communication skills.
  • Ability to multitask effectively.
  • Proficient in technology and comfortable with new tools.
  • Proven success in building relationships.
  • Team-oriented with a positive attitude.
  • Ability to work outside of job description as needed.
  • Advanced Degree in Social Work or mental health preferred.
  • Bilingual in English and Spanish or Haitian-Creole preferred.
