Overview

As a Community Case Manager at Pine Tree Society, you will provide resources and support to meet the individualized needs of individuals with intellectual disabilities and other developmental disorders.

In Short

Requirements

  • Bachelor's degree in social work or related field.
  • One or more years of experience with developmental disabilities.
  • Case management experience preferred.
  • Ability to develop goals and services for individuals.
  • Strong communication and advocacy skills.
  • Commitment to community inclusion and independence.
  • Willingness to participate in various appointments and meetings.
  • Proficiency in entering case notes accurately.
