In Short

• Support members’ health and wellness through comprehensive assessments.
• Identify needs and guide members and their families to essential resources.
• Employ various strategies to manage health issues telephonically.
• Resolve barriers to effective care and monitor patient progress.
• Make decisions regarding work priorities and follow established guidelines.
• Work remotely from home while residing in Illinois.
• Typical work hours are Monday – Friday, 8:00am – 5:00pm CST.
• Must be bilingual in English and Spanish without limitations.
• Experience in case management is required.

Requirements

• Bachelor’s degree in health or social service-related field or LPN licensure.
• 2 or more years of case management experience.
• Proficient in Microsoft Word, Excel, and Outlook.
• Experience with electronic information applications/software programs.
• Preferred: Certified Case Manager (CCM) and Master's degree.
• Knowledge of community health and social service agencies.
• Experience with health promotion, coaching, and wellness.
• Experience with patient education for adult acute care.
