Requirements

• Licensed Registered Nurse (RN) in Illinois.
• Minimum 3 years of clinical acute care experience.
• Bilingual in English and another language (Spanish, Polish, or Creole).
• Comprehensive knowledge of Microsoft Office applications.
• Preferred: Bachelor's degree in Nursing (BSN).
• Experience with case management and patient education.
• Managed care experience is a plus.
• Prior experience with Medicare & Medicaid recipients.
• Knowledge of community health resources.
• Ability to work from a dedicated space free of interruptions.
