Care Manager, Telephonic Nurse 2 - Remote

The Care Manager, Telephonic Nurse 2, in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members.

In Short

• Assess and evaluate members' needs in a telephonic environment.
• Guide members and families toward appropriate resources.
• Manage physical, environmental, and psycho-social health issues.
• Create member care plans and monitor progress towards outcomes.
• Requires independent determination of appropriate courses of action.
• Minimum 3 years acute care experience required.
• Must be a licensed Registered Nurse (RN) in Florida.
• Bilingual English/Spanish preferred.
• Experience with pediatric care and case management is desirable.
• Work from home with occasional travel to Humana's offices.

Requirements

• Licensed Registered Nurse (RN) in Florida with no disciplinary action.
• 3+ years acute care experience, with 2+ years in Pediatric care.
• 1-2 years of care management experience.
• Comprehensive knowledge of Microsoft Office applications.
• Experience with case management and discharge planning preferred.
• Managed care experience is a plus.
• Certification as a Case Manager (CCM) is preferred.
