Overview

The Telephonic Nurse Case Manager will provide a comprehensive, holistic approach to case management for TRICARE beneficiaries, ensuring coordination of care and support throughout the health continuum.

  • Support Care Coordinators for beneficiaries not under case management.
  • Work from home with specific internet requirements.
  • Attend mandatory training during the first four weeks.
  • Occasional travel to Humana's offices may be required.
  • Full-time position with a salary range of $71,100 - $97,800 per year.

Requirements

  • U.S. citizenship required.
  • Current, valid, and unrestricted RN license.
  • Minimum of Associate’s degree in Nursing.
  • 2 years of case management experience.
  • Active CCM designation required within the first year of hire.
  • 3+ years of clinical nursing or managed care experience.
  • Proficiency in Microsoft Office (Word, Excel, Outlook).
  • Bachelor’s or Master’s in Nursing preferred.
  • Experience in Utilization Management a plus.
  • Bilingual in Spanish and English preferred.
