Wellness Coach - Remote

Overview

The Wellness Coach interacts with members to address care needs, promote wellness, and improve overall health outcomes as part of a Care Team.

In Short

• Connect with members to develop and support health care.
• Establish trusting relationships with members and caregivers.
• Meet with the team to review goals and targets.
• Document member demographics and healthcare information.
• Answer calls from members, providers, and other resources.
• Conduct outreach calls to describe benefits of programs.
• Engage with members to ensure understanding of care terms.
• Report complaints and identify corrective actions.
• Perform other duties as assigned.

Requirements

• Associate’s degree in relevant field or equivalent experience.
• 2 years’ experience in healthcare with chronic diseases.
• Experience in managed care or account management preferred.
• Experience working with Medicare and Medicaid preferred.
• Previous experience in a metrics-driven environment preferred.
