Wellness Coach - Remote

Overview

The Wellness Coach at Healthmap’s Clinical Services interacts with members to address care needs, promote wellness, and improve overall health outcomes by managing their caseload through various communication platforms.

In Short

• Connect with members to develop and support health care.
• Establish trusting relationships with members and caregivers.
• Meet with the team to review goals and targets.
• Document member demographics and healthcare information.
• Answer inbound and outbound calls to support objectives.
• Conduct outreach calls to enroll eligible patients.
• Engage with members to explain care terms.
• Report complaints and identify corrective actions.
• Perform other assigned duties.

Requirements

• Associate’s degree in relevant field or equivalent experience.
• 2 years’ experience in healthcare, preferably with chronic diseases.
• Experience in managed care or account management preferred.
• Experience with Medicare and Medicaid preferred.
• Previous experience in a metrics-driven environment preferred.
