The RN Case Manager primary responsibility is to create, implement, and monitor treatment plans to assist patients/members in meeting their healthcare goals.

Contact with patients, families, and healthcare providers.

Monitor health status and treatment plan effectiveness.
Identify services and resources for members.
Develop and implement Case Management goals.
Utilize technology for cost-effectiveness evaluation.
Ensure accurate and timely reporting.
Review medical records for necessity of services.
Interpret health plans and coordinate care.
Requirements
5+ years of clinical nursing experience.
Bachelor’s degree or equivalent experience.
Graduate of an accredited nursing school.
RN State Licensure.
Knowledge of medical terminology and treatment plans.
