Overview

The Intake Case Manager plays a vital role within GMHC’s Housing team, supporting individuals and families experiencing housing instability, including those with histories of homelessness. This position is responsible for coordinating comprehensive intake assessments, facilitating referrals to critical services, and supporting clients in accessing entitlements, healthcare, and permanent housing solutions.

In Short

  • Collaborate with HRA to verify client eligibility and ensure enrollment in GMHC services.
  • Conduct comprehensive intake assessments to determine needs and eligibility for public benefits.
  • Maintain a caseload of up to twenty-five clients across New York City.
  • Schedule and conduct monthly home visits to assess housing stability.
  • Educate clients on tenant rights and responsibilities.
  • Monitor and collect monthly rent and utility payments.
  • Maintain accurate client records and documentation.
  • Stay current on housing policies and best practices through training.
  • Represent GMHC with professionalism and compassion.
  • Perform additional responsibilities as assigned by the supervisor.

Requirements

  • Commitment to empathy and understanding in client interactions.
  • Knowledge of housing subsidy programs and benefits.
  • Experience in case management strategies for housing stability.
  • Strong active listening and communication skills.
  • Experience working with vulnerable populations.
  • Ability to work independently and in teams.
  • Bilingual (English/Spanish) skills are a plus.
  • High School Diploma or GED required; degree in related field preferred.
  • Five years of experience in social services related to housing instability.
  • Intermediate proficiency in Microsoft Excel and Office Suite.
