Overview

The Health Navigator connects individuals and families to affordable health care coverage by providing enrollment assistance, education, and outreach through the New York State of Health (NYSOH) Navigator Program.

In Short

  • Conduct outreach to promote the NYSOH Navigator Program.
  • Deliver presentations to educate about health insurance options.
  • Assist clients with health insurance enrollments and renewals.
  • Maintain accurate documentation of client interactions.
  • Participate in program reviews and data quality checks.
  • Support clients in gathering necessary documentation.
  • Facilitate referrals for additional services.
  • Collaborate with external partners to improve accessibility.
  • Adhere to agency and funder policies.
  • Participate in team meetings and training.

Requirements

  • Strong understanding of healthcare access barriers.
  • Experience in a fast-paced, data-driven environment.
  • Exceptional active listening and interpersonal skills.
  • Experience with vulnerable populations.
  • Bilingual (English/Spanish) fluency.
  • Ability to work independently and collaboratively.
  • Commitment to culturally responsive services.
  • High School Diploma or equivalent required.
  • Must obtain NYSOH Navigator certification.
  • Experience with Microsoft Excel and Office Suite.
