The Case Manager is responsible for coordination of care and service delivery for an assigned group of patients, promoting inter-dependent communication and collaboration with healthcare teams.

Requirements

  • Associate's Degree in Nursing (RN).
  • 1-3 years of clinical healthcare experience.
  • Licenses: RN in Arkansas, Mississippi, Tennessee.
  • Accredited Case Manager certification preferred.
  • Excellent oral and written communication skills.
  • Ability to collaborate with healthcare professionals.
  • Strong facilitation and planning skills.
  • General computer skills; advanced skills preferred.
  • Independent judgment and critical thinking skills.
  • Physical ability to perform job functions.
