Care Manager (Social Work) - Remote

Overview

The Care Manager will develop, assess, and facilitate complex care management activities for members with mental and behavioral health needs, providing high quality, cost-effective healthcare outcomes.

In Short

• Develops personalized care plans for members with mental health and substance use disorder needs.
• Conducts evaluations via phone or in-home visits to assess member needs.
• Coordinates care between members, families, and healthcare providers.
• Monitors and adjusts care plans based on member status and outcomes.
• Educates members and families on treatment options and healthcare benefits.
• Collaborates with community resources and specialists for care access.
• Maintains compliance with state, federal, and payer regulations.
• Requires approximately 75% travel for member home visits.
• Must have a Master's degree in Behavioral Health or Social Work.
• Licensed Master's Behavioral Health Professional or RN required.

Requirements

• Master's degree in Behavioral Health or Social Work or equivalent nursing degree.
• 2-4 years of related experience in care management.
• Licensed Master's Behavioral Health Professional or RN with BH experience.
• Strong communication and organizational skills.
• Ability to work independently and as part of a team.
