The Care Manager (RN) for OB will develop and facilitate complex care management activities to provide high-quality healthcare outcomes for members, focusing on personalized care plans and education.

Requirements

• Experience in care management or healthcare settings.
• Knowledge of healthcare regulations and compliance.
• Proficient in developing care plans.
• Familiarity with community resources and support services.
• Ability to collaborate effectively with healthcare teams.
