Overview

The Care Manager (RN) is responsible for developing and facilitating care management activities for members with physical needs, ensuring high-quality healthcare outcomes through personalized care plans and education.

Requirements

• Degree from an Accredited School of Nursing or Bachelor's in Nursing.
• 2 – 4 years of related experience.
• Registered Nurse licensure required.
• Pediatric experience preferred for Florida's CMS.
• Ability to travel locally up to 80%.
