RN Care/Case Manager - Remote

Overview

Join our caring community as a Care Navigator – Registered Nurse, where you will engage patients and implement interventions to enhance their health and access to care.

In Short

• Work within an interdisciplinary care integration team.
• Conduct transitions of care management for patients.
• Provide triage guidance and support to team members.
• Develop care plans using the 5Ms Geriatric best practice framework.
• Identify barriers to patient engagement and provide education.
• Serve as a liaison between patients and care providers.
• Lead interdisciplinary team meetings and document patient encounters.
• Participate in team training and maintain patient confidentiality.
• Work in a hybrid setting with onsite patient engagement.
• Must reside in the Las Vegas area and work a 40-hour week.

Requirements

• Registered Nurse (RN) license.
• Minimum of 4 years of healthcare experience.
• Experience in care/case management preferred.
• Bilingual in English and Spanish is a plus.
• Strong clinical acumen and organizational skills.
• Ability to multi-task in a fast-paced environment.
• Excellent communication and interpersonal skills.
• Critical thinking and problem-solving abilities.
• Reliable transportation and valid driver's license required.
• Compliance with TB screening program.
