Patient Services Coordinator LPN Home Health - Remote

Overview

The Patient Services Coordinator-LPN is responsible for scheduling visits and maintaining communication among field staff, patients, and physicians to ensure proper care coordination and continuity of care.

• Schedule and manage patient visits.
• Communicate with field staff and physicians.

• Perform initial reviews of weekly schedules.

Requirements

• Must be a Licensed Professional Nurse or Licensed Vocational Nurse.
• At least 1 year of home health experience required.
• Prior packet review/QI experience preferred.
• Coding certification is preferred.
• Valid state driver’s license and automobile liability insurance required.
• Excellent communication and organizational skills needed.
• Ability to manage multiple assignments effectively.
• Current state license in the state of employment is necessary.
