Registered Nurse - Chronic Care Management - Remote

Overview

The Cadence Health team seeks a Registered Nurse to support our newly established Chronic Care Management Program. The RN will provide care, education, and support to our CCM patients to better manage their conditions.

In Short

• Continuously monitor patient vitals, symptoms, and lab results.
• Create and manage personalized care plans.
• Address patient concerns via phone and text.
• Conduct virtual follow-up appointments.
• Document clinical interactions thoroughly.
• Assist in developing workflows for the CCM program.
• Ensure exceptional patient care interactions.
• Collaborate with the team to scale care delivery.

Requirements

• Multi-state RN Compact State Licensure.
• Associate Degree in Nursing Science.
• 5+ years of clinical experience in a chronic care management program.
• Previous experience leading or building a CCM program.
• Excellent clinical acumen and communication skills.
• EMR experience, preferably in Athena and EPIC.
• Strong collaboration with cross-functional partners.
• Technical fluency with multiple platforms.
