PRN Registered Nurse for Chronic Disease Management - Remote

Overview

The Cadence Health team seeks a PRN Registered Nurse that will be responsible for appropriately monitoring and triaging patients based on vitals and alerts; supporting the management of patient treatment plans and medications in collaboration with Cadence NPs and the patient's Physician.

In Short

• Provide relief for regularly scheduled staff during time off.
• Cover shifts during the day, nights, weekends, and holidays.
• Respond to inbound patient clinical questions and escalations.
• Follow standardized triage protocols for incoming calls.
• Support clinical documentation for the patients’ Cadence care team.
• Monitor patient vitals, symptoms, and labs.
• Ensure exceptional healthcare interactions with Cadence.
• Document patient case notes in the Cadence platform.
• Provide additional administrative support as needed.
• Work independently in a less structured environment.

Requirements

• RN compact multi-state license and additional state license.
• 3+ years of experience treating patients with chronic conditions.
• Passion for patient/customer experience and digital innovation.
• Independent thinker/operator.
