Overview

The Arizona Long Term Care ALTCS Case Manager is responsible for managing and coordinating care for members in the AHCCCS population, ensuring their needs are met through comprehensive assessments and service planning.

In Short

Requirements

• Bilingual preferred in some assignments.
• Ability to work collaboratively with healthcare teams.
• Valid driver's license for travel.
