Are you a Registered Nurse (RN), Licensed Practical Nurse (LPN), or Social Worker looking for a part-time PRN position? We have an exciting opportunity for someone passionate about providing care and making a difference in the lives of others!

In Short

• Conduct assessments and develop care plans tailored to client needs
• Coordinate care and resources to support clients in achieving their goals
• Work collaboratively with families, caregivers, and healthcare providers
• Monitor progress and provide updates to ensure clients receive exceptional care
• Develop and maintain an accurate and thorough knowledge of all program requirements
• Comply with HIPAA and agency confidentiality policies
• Complete and attest to accuracy of payroll records
• Professional with a positive approach to enhance engagement rate
• Strong communicator, able to write clear documentation within 24 hours
• Self-Directed, Organized, Team Player

Requirements

• Degree in social work, RN, or LPN, preferred
• Experience with community resources related to social determinants of health
• Remote abilities with high-speed internet access
• In-person assessments required monthly
• Computer proficiency in Microsoft Word, Excel, and Outlook
• Ability to pass a Criminal Background Check, Drug Screen, and DMV Check
