Overview

Are you a Registered Nurse (RN), Licensed Practical Nurse (LPN), or Social Worker looking for a part-time PRN position? We have an exciting opportunity for someone passionate about providing care and making a difference in the lives of others!

In Short

Conduct assessments and develop care plans tailored to client needs
Coordinate care and resources to support clients in achieving their goals
Work collaboratively with families, caregivers, and healthcare providers
Monitor progress and provide updates to ensure clients receive exceptional care
Provide population health services to assigned clients on an ongoing and emergency basis
Develop and maintain an accurate knowledge of all program requirements
Comply with HIPAA and agency confidentiality policies
Communicate with supervisor on a daily basis to report concerns
Participate in staff and training meetings for professional development
Demonstrate professionalism and commitment to customer service

Requirements

Degree in social work, RN, or LPN, preferred
Experience with community resources related to social determinants of health
Remote abilities with high-speed internet access
Computer proficiency in Microsoft Word, Excel, and Outlook
Ability to pass background checks as required by company policy
