Overview

This role serves as the primary contact for members to coordinate all care needs across various service delivery systems and community supports, requiring frequent travel within the assigned territory in Delaware.

In Short

  • Coordinate care for members with chronic and complex healthcare needs.
  • Conduct in-home and nursing facility visits for assessments.
  • Develop individualized care plans with members and caregivers.
  • Authorize LTSS services based on comprehensive needs assessments.
  • Facilitate transitions between different care settings.
  • Educate members on healthcare needs and available resources.
  • Document all case management services in the electronic health record.
  • Ensure compliance with state and federal privacy practices.
  • Travel within the assigned territory for in-person visits.
  • Work collaboratively with healthcare teams to meet member needs.

Requirements

  • Bachelor's or Master's degree in Social Work or related field.
  • Experience in long-term care, home health, or public health.
  • Strong understanding of Medicaid and Medicare systems.
  • Proficiency in documentation and case management.
  • Ability to work flexible hours to meet member needs.
  • Reliable transportation for travel within the territory.
  • Experience with behavioral health and developmental disabilities preferred.
  • Certification as a Case Manager is a plus.
  • Knowledge of cultural competency in healthcare.
  • Experience with electronic documentation systems.
