Requirements

• Active Registered Nurse (RN) License in the state of employment.
• 2+ years of recent direct care experience with elders/adults with disabilities.
• Background in case management, care planning, and assessments preferred.
• Valid driver’s license and auto insurance required.
• Proficient in MS Office and case management systems.
• Bilingual skills are a plus.
