In Short

• Serve as the telehealth concierge and first point of contact for patients.
• Conduct high volume of intake appointments to understand patient goals.
• Match patients with appropriate Bold physicians or care pathways.
• Provide guidance on health plan benefits and cost expectations.
• Manage telehealth appointments and ensure technical support for patients.
• Conduct outreach to patients for care initiation and follow-ups.
• Troubleshoot patient issues and resolve barriers to care.
• Collaborate with clinical teams to improve patient flow and experience.
• Participate in process improvement initiatives.

Requirements

• 1–3+ years of direct patient care or care coordination experience.
• Strong verbal and written communication skills.
• Experience achieving measurable KPIs.
• Ability to manage high volume patient interactions.
• Problem-solving orientation in a fast-moving environment.
• Comfort with CRM platforms and telehealth software.
