Overview

The Care Coordinator serves as a patient advocate and point of contact for care recommendations, working closely with various disciplines to ensure patients receive optimal support and care.

Requirements

• Bachelor’s degree preferred; High School diploma required.
• Two years of customer service experience, preferably in healthcare.
• Proficiency with software platforms and EMRs.
• Knowledge of medical/psychiatric diagnosis and treatment processes.
• Strong communication and organizational skills.
• Ability to work cooperatively with all staff.
• Open to change and willing to support department goals.
• Professional demeanor and attire.
• Ability to influence departments and provide technical guidance.
• Ability to maintain a calm attitude in difficult situations.
