Overview

The Specialty Care Manager RN is responsible for facilitating communication among patients, families, and healthcare teams to ensure quality and efficient care delivery while managing patient caseloads and addressing potential barriers to care.

Requirements

• Registered Nurse license issued by the state of practice.

• Excellent oral, written, and interpersonal skills.
• Critical thinking and problem-solving abilities.
• Proficient in Microsoft Office and Epic EMR.
• Ability to work independently and in a team environment.
• Knowledge of patient care principles across the lifespan.
• Ability to handle multiple responsibilities effectively.
• Demonstrated ability to work collaboratively with healthcare professionals.
