Overview

The Kidney Care Advocate I is responsible for collaborating with care teams to provide education and support to patients with kidney disease, ensuring a positive experience and facilitating access to care services.

  • Collaborate with internal and external care teams.
  • Educate patients on dialysis options and kidney care services.
  • Coordinate patient admissions and referrals.
  • Maintain communication with healthcare providers and teams.
  • Assist in resolving barriers to home therapy training.
  • Promote Kidney Care Advocate services to the community.
  • Document patient interactions accurately.
  • Support educational programs for healthcare staff.
  • Travel between clinics as required.
  • Perform other related duties as assigned.

Requirements

  • High school diploma or GED required; college degree preferred.
  • Minimum of one-year experience in teaching or sales.
  • Dialysis experience preferred but not required.
  • Excellent verbal, written, and presentation skills.
  • Proficient in Microsoft Office and CRM systems.
  • Ability to work autonomously and manage multiple cases.
  • High emotional intelligence and ability to relate to diverse populations.
  • Team player with a results-oriented mindset.
  • Must be able to travel locally and regionally.
  • Willingness to work evenings and weekends as needed.
