Intake Specialist - Remote

Overview

Zócalo Health is seeking an organized and compassionate Intake Specialist to serve as the initial point of contact for patients accessing our care services. This role is essential in ensuring that patients receive seamless support as they begin their healthcare journey with Zócalo Health.

In Short

• Complete scheduled intake appointments with patients.
• Update and maintain the county-based case management tracker.
• Create care planning accounts for new patients.
• Schedule follow-up appointments with case managers.
• Attend outreach events and represent Zócalo Health.
• Collect and document patient interest at community events.
• Document patient information in electronic health records.
• Escalate issues to appropriate team members.
• Collaborate with Clinical Operations teams for coordinated care.
• Support outreach efforts with documentation for claims submission.

Requirements

• 1-3 years of experience in healthcare or customer service.
• Experience with patient intake and scheduling preferred.
• Fluent in English and Spanish.
• Exceptional interpersonal and communication skills.
• Detail-oriented with strong organizational skills.
• Comfortable with technology and EHR systems.
