Overview

The Regional Advanced Practice Provider - Geriatric Resources role focuses on enhancing health care experiences for older adults through personalized, home-based care. This position is part of Mass General Brigham’s Population Health team, which aims to deliver tailored care to meet the unique needs of each individual.

In Short

  • Provide home-based geriatric assessment and care management for older adults.
  • Conduct in-home assessments in collaboration with a GRACE Social Worker.
  • Develop and implement individualized care plans using GRACE care protocols.
  • Engage patients and caregivers in health education and chronic disease management.
  • Coordinate care with primary care providers and the GRACE interdisciplinary team.
  • Work in a hybrid model with both in-home visits and remote work.
  • Full-time position with 36 hours of clinical time and 4 hours of administrative time weekly.
  • Expected to conduct up to 10 home visits per week.
  • Requires a Master's Degree in Nursing or Physician Assistant.
  • Must hold a valid MA State License as a Nurse Practitioner or Physician Assistant.

Requirements

  • Master's Degree in Nursing or Physician Assistant required.
  • Registered Nurse [RN] with MA State License required.
  • Experience as a Nurse Practitioner preferred (0-1 year).
  • Ability to take medical histories and assess medical conditions.
  • Strong communication skills for effective patient relationships.
  • Ability to react calmly and effectively in emergency situations.
  • Experience in geriatric care is advantageous.
  • Ability to maintain quality control standards.
  • Skills in developing individualized care plans.
  • Ability to work collaboratively in an interdisciplinary team.
