Launch Advanced Practice Provider - Remote

Overview

As a Launch Advanced Practice Provider at Imagine Pediatrics, you will support the mission of increasing safe days at home for medically complex children by managing their care plans and coordinating care across the healthcare system.

In Short

• Manage a panel of complex pediatric patients.
• Collaborate with pediatric RNs and other care team members.
• Educate and empower parents on care plans.
• Leverage virtual platforms for patient visits.
• Partner with a multidisciplinary team for coordinated care.
• Support new market launches during go-live phases.
• Provide additional support to existing markets as needed.

Requirements

• Minimum 5 years’ experience with a pediatric population required.
• Completion of an accredited program for Nurse Practitioner or Physician Assistant.
• Unrestricted NP or PA state license in Texas, New York, and Florida.
• Medicaid enrollment in New York and/or Texas is preferred.
• Willingness to become cross-licensed in new states as needed.
