Overview

As a Nurse Practitioner at Oak Street Health, you will provide effective and equitable primary care to adults on Medicare, focusing on building patient relationships and collaborating with a healthcare team.

In Short

• Provide exceptional primary care services.

Requirements

• Master's degree in Nursing or equivalent.
• National certification as a Nurse Practitioner.
• Active state Nurse Practitioner license.
• Active DEA license.
• Experience in primary care preferred.
• Understanding of HCC documentation and ICD-10 coding.
• Strong teamwork and collaboration skills.
• Desire to innovate in population health.
• Bilingual proficiency is a plus.
