Overview

The Regional Social Worker will provide home-based geriatric assessment and care management for older adults, ensuring comprehensive and coordinated care that aligns with their goals and preferences.

Requirements

• Master's Degree in Social Work or Mental Health & Behavioral Medicine.
• Preferred LCSW, LICSW, or LMHC state license.
• Clinical experience in a medical setting (2-3 years).
• Proficient clinical judgment and application of professional values.
• Ability to formulate assessments, treatment plans, and document in a timely manner.
• Sound clinical judgment and innovation in advocating for clients.
• Knowledge of internal and external resources.
• Ability to coordinate and communicate effectively with patients and interdisciplinary teams.
• Engage in quality improvement projects using data to measure progress.
• Facilitate skill-based groups.
