Overview

The Case Management Team Lead is responsible for coordinating patient care services, conducting evaluations, and advocating for necessary services and entitlements while overseeing the day-to-day operations of the department.

Requirements

• Bachelor's Degree in Nursing or Master's Degree in Social Work required.
• 5-7 years of experience in Case Management, Utilization Review, or Discharge Planning.
• Licenses: Licensed Independent Clinical Social Worker (LICSW) preferred.
• Registered Nurse (RN) preferred.
• Strong problem-solving and prioritization skills.
• Ability to establish rapport with patients and staff.
• Knowledge of healthcare and managed care preferred.
• High level of service delivery and initiative.
• Understanding of diagnostic criteria for dual conditions.
• Excellent listening and communication skills.
