Requirements

• Active Pharmacy License in the state of employment.
• Not on the DEA Excluded Parties List.
• Immunization Certification from an accredited organization.
• Listed as the ‘pharmacist in charge’ on state license.
• Submission of required documents for PMP access.
• Bachelor of Science in Pharmacy or Pharm. D. preferred.
